IV · Malpractice insurance

Current certificate of insurance. Coverage limits must reasonably cover the procedures you perform.

V · Injector roster

A spreadsheet or PDF with every person who performs medical procedures at your spa — their name, license number, and the procedures they perform. We confirm each license is active and the procedures fall within their legal scope.

VI · Attestation

By signing below, you affirm everything above is accurate as of today. If anything material changes, you commit to telling us within 14 days.
